Job Description

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must show knowledge of the principles of growth and development across the life span and possess the ability to assess data reflective of the patient's status. They will interpret the necessary information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide essential care to adult and geriatric patients.

Responsibilities

  • Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to RN and other team members.
  • Contribute to creating a developmentally age-appropriate and culturally relevant plan of resident care, involving residents/families and the healthcare team.
  • Recommend revisions to the plan of care after evaluating resident outcomes and determining the need for referrals.
  • Maintain current knowledge and competency in Nursing Practice; participate in mentoring/orientation of staff.
  • Under the direction of the RN, provide leadership in the professional practice setting, considering factors such as safety and operational impact.
  • Acknowledge occupational exposure to bloodborne pathogens and perform required tasks with necessary precautions.

Required Experience / Certifications / Licensure

  • Current NY LPN Licensure.
  • Basic Life Support (BLS) certification through AHA or RQI.
  • Minimum of 1 year experience serving in an LPN capacity at a Long-Term Care facility.

Schedule

This position requires part-time availability during evening shifts, with a rotational schedule of 2 shifts in one week and 3 shifts in the following week. Weekend and holiday shifts are required.
