Job Description

At our hospice, we are committed to providing compassionate, high-quality end-of-life care to our patients and their families. As a leader in hospice care, we believe in treating every patient with dignity and respect in their preferred place of care, usually in their home. We are seeking a dedicated and compassionate Hospice Registered Nurse (RN) to join our interdisciplinary team.

Responsibilities

• Perform comprehensive assessments of patients' needs and coordinate individualized care plans.
• Provide pain and symptom management consistent with the hospice plan of care.
• Educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy.
• Collaborate with the interdisciplinary team including physicians, social workers, chaplains, and volunteers.
• Participate in on-call rotation and maintain accurate and timely documentation.

Required Experience / Certifications / Licensure

• Current Registered Nurse (RN) license in the state of Utah or compact state RN license.
• Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care.
• Strong clinical assessment skills and excellent communication, interpersonal, and organizational skills.
• Compassionate demeanor with the ability to support patients and families during difficult times.
• Valid driver's license and reliable transportation required for home visits.
• Current BLS Certification.
