Schedule

Day shift, 5 days a week, with variations in start times between 07:00 and 08:30. Weekends required every other weekend or 4 weekend days in a 4-week schedule.

Job Description

We are seeking a dedicated Case Manager RN to join our acute care hospital team. The ideal candidate will have acute care experience, BLS certification, and be well-versed in Epic charting systems.

Responsibilities

• Provide case management and discharge planning for patients.
• Coordinate care with interdisciplinary teams and ensure patient needs are met.
• Utilize Epic for documentation and patient management.

Required Experience / Certifications / Licensure

• Active Alaska RN license.
• Minimum of 1 year of relevant acute care experience.
• Current BLS certification.
• ACM certification preferred.
