Schedule

Shift: 5x8 Days, with the possibility of varied start times between 07:00 and 17:00. Weekends are required every other weekend or 4 weekend days within a 4-week schedule.

Job Description

The Case Manager RN will be responsible for coordinating patient care and planning for discharges. The role requires collaboration with interdisciplinary teams and involves managing patient assessments and developing care plans.

Responsibilities

• Discharge planning and care coordination
• Utilize Epic for patient charting and documentation
• Conduct needs assessments and plan care for patients
• Ensure compliance with CMS and HIPAA guidelines

Required Experience / Certifications / Licensure

• Current AK RN license (required for Auto Offer)
• BLS certification (required)
• ACM certification (preferred)
• 1 year of acute care nursing experience
• Epic experience is mandatory
