Job Description

The Case Manager plays a crucial role in utilization reviews, resource management, discharge planning, treatment plan management, and financial management while completing necessary medical record documentation. The position focuses on promoting patient and employee safety and adheres to the Select Medical Way and Core Values of delivering superior quality in all services provided.

Responsibilities

• Identify patients' problems affecting their ability to adapt to treatment plans.
• Assist patients and families in utilizing hospital resources to remove barriers to healthcare situations.
• Facilitate patient and family participation in decision-making regarding their care plans.
• Collaborate with the interdisciplinary team to manage and direct clinical plans for safe and timely discharge.
• Maintain accurate and thorough medical documentation in accordance with regulations.
• Participate as a team member to improve operations and identify facility-wide needs.
• Ensure timely completion of utilization review and adherence to payor requirements.
• Document care plans and progress notes as required by policies and regulations.

Required Experience / Certifications / Licensure

• Current licensure in a clinical discipline per state guidelines (RN, RT, or SW preferred).
• Working knowledge of the insurance industry and government reimbursement processes.
• Minimum of 2 years experience in Case Management and Discharge Planning in an Acute Care setting.
