Job Description

Fall River Health Services is looking for a Physical Therapist (PT) to support the treatment of illness and/or injury through physical exercises and therapies, assisting in the rehabilitation and restoration of body functions to promote maximum client wellness and independence. This role involves providing therapy programs for patients of all ages with developmental deficits, physical injury, or illness.

Responsibilities

• Coordinate PT services, providing assigned clients with direct therapeutic care and teaching rehabilitation skills.
• Provide preventive and rehabilitative PT procedures individualized to the client according to age-specific needs.
• Develop and revise a plan of care based on continual reassessment of client/family needs, history, diagnosis, and goals.
• Perform and monitor treatments involving physical agents such as heat, water, and electricity.
• Formulate and implement appropriate teaching plans while communicating significant clinical findings.
• Document all skilled and medically necessary care thoroughly and accurately in compliance with applicable standards.
• Supervise Physical Therapy Assistants and Home Health Aides in accordance with Federal/State laws and agency policy.
• Participate in staff meetings, case conferences, and customer service programs as required.
• Deliver a high level of customer service to all internal and external customers.

Required Experience / Certifications / Licensure

Qualified candidates must hold a valid physical therapist license and possess relevant clinical experience.
