Day shift, 5x8-hour schedule (08:00 - 16:00).

Job Description

We are seeking a Registered Nurse with Case Management experience in an acute care setting. The ideal candidate should have experience using Cerner and preferably have knowledge of Wellsky and PASRR entry systems. You will be responsible for managing patient care, ensuring effective communication among interdisciplinary teams, and providing support throughout the patient care continuum.

Responsibilities

• Manage patient cases and coordinate care plans.
• Work within interdisciplinary teams to provide comprehensive care.
• Utilize Cerner and other required systems for patient documentation.
• Advocate for patients and facilitate discharge planning.

Required Experience / Certifications / Licensure

• Minimum of 5 years of hospital case management experience is required.
• Experience in acute care case management.
• Cerner experience is mandatory.
• First-timers are accepted for consideration.
• Travel experience is required.
• Certifications are not required.
