Job Description

The Physical Therapist ensures that physical therapy services are administered in a timely, accurate, and reliable manner. This role provides evaluations and treatments, along with associated clinical documentation, while maintaining service delivery standards and corporate compliance requirements.

Responsibilities

• Assess patient strengths, limitations, and needs using standardized measurement and evaluation tests.
• Develop and implement individualized treatment goals based on patient assessments.
• Administer and interpret clinical physical therapy services, adjusting treatment plans as needed.
• Maintain accurate and timely patient treatment records in electronic health systems.
• Establish and maintain effective working relationships with patients, families, and interdisciplinary team members.
• Provide education to patients and families regarding treatment plans and self-care techniques.
• Participate in discharge planning and referrals for durable medical equipment.
• Supervise and provide direction to Physical Therapist Assistants during treatment sessions.
• Engage in departmental and organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Graduate of an accredited school of physical therapy with a bachelor’s degree or equivalent.
• Current license or eligibility to be licensed as a Physical Therapist in the State of New York.
• BLS Certification (AHA, HSI, or American Red Cross).
• Previous experience in an approved medical facility.
