Job Description

We are seeking a skilled Home Health Registered Nurse to provide high-quality patient care in a home health setting. This role involves assessing patients' needs, implementing appropriate interventions, and ensuring comprehensive documentation of care. You will work with a variety of patients including those recovering from surgery, managing chronic conditions, and requiring rehabilitation support.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Provide education to patients and families on health management and medications.
• Utilize various equipment and devices to assist in patient mobility and safety.
• Document patient care accurately and timely, following OASIS guidelines.
• Manage home health visits, aiming for a minimum of 6 visits per day.
• Collaborate with interdisciplinary team members for comprehensive patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of home health nursing experience required.
• Current BLS certification is required.
• State licensure as a Registered Nurse in Florida or a compact license.
• OASIS training preferred.
• Must have valid driver's license and reliable transportation for home visits.
• COVID-19 vaccination or exemption form on file required.
• Flu shot or declination required; adherence to safety protocols is mandatory.
