Job Description

The Case Manager RN is responsible for discharge planning and care coordination within an acute care hospital setting. This position requires substantial experience with Epic charting and acute care environments.

Responsibilities

• Coordinate discharge planning and needs assessment for patients.
• Utilize Epic for charting and documentation purposes.
• Collaborate with interdisciplinary teams to ensure optimal patient care.
• Conduct needs assessments and develop a plan of care for patients.

Required Experience / Certifications / Licensure

• Active Alaska RN license required for auto-offer.
• BLS certification required.
• ACM certification preferred.
• Minimum 1 year of acute care experience.
• Epic experience is mandatory.
