This is a 13-week contract position.

Schedule

8:00 AM - 5:00 PM, Monday - Friday; around 8/9 AM start. 40 hours/week.

Job Description

The Physical Therapy Assistant is responsible for providing physical therapy treatments under the supervision of a qualified Physical Therapist in accordance with the plan of care. The Home Health Physical Therapist Assistant will utilize therapeutic exercise, modalities, and activities to achieve the highest level of function possible within the home environment.

Responsibilities

• Provide physical therapy treatments as directed by the Physical Therapist.
• Utilize therapeutic exercises and modalities to enhance patient function.
• Work collaboratively with the healthcare team to ensure patient care is aligned with the plan of care.
• Document patient progress and report any changes to the supervising therapist.
• Drive comfortably in varying weather conditions, including winter weather.

Required Experience / Certifications / Licensure

• 1-2 years current Physical Therapy Assistant experience in Home Health is required.
• Experience with age groups ranging from Pediatrics to Geriatrics.
• Previous travel experience is highly preferred.
• Home Health and/or Hospice experience is required.
• Active American Heart Association BLS certification.
• Active Physical Therapy Assistant practice license from the New Hampshire Office of Allied Health Professionals.
• All licenses and certifications must be current at the time of submission.
