Part Time Evenings (2x8s/3x8s), every other weekend required. Must be available for full shift orientation on a Tuesday or Thursday.

Job Description

The Licensed Practical Nurse (LPN) performs direct and indirect designated resident care under the direction of licensed nursing personnel. This role involves meeting the physical, emotional, and psychosocial needs of patients while adhering to established policies and procedures.

Responsibilities

• Ensure resident's Activities of Daily Living (ADLs) are met, including hygiene, grooming, and feeding.
• Maintain resident safety and cleanliness in accordance with care plans.
• Document resident care according to protocols.
• Report any changes in resident status to the Charge Nurse promptly.
• Assist with rehabilitative needs, including ambulation and the use of assistive devices.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• BLS certification (AHA or RQI only)
• Minimum of 1 year experience as an LPN, preferably in a long-term care (LTC) or CNA capacity.
