Job Description

The Case Manager - Registered Nurse is responsible for the comprehensive management of patients, collaborating with medical providers and multidisciplinary teams to maximize healthcare outcomes. The role involves coordinating care conferences, managing referrals, and serving as a key advocate for patients and their families.

Responsibilities

• Manage active SEARHC patients and coordinate with medical providers to establish individualized care goals.
• Conduct multi-disciplinary care conferences for high-risk patients.
• Manage panel acute and chronic care needs, continuously reviewing electronic health records (EHR).
• Track referral progress and coordinate appointments within SEARHC and external facilities.
• Perform nursing duties including assessment, documentation, and patient education.
• Participate in case management reviews with a multi-disciplinary team.
• Facilitate specialty clinic referrals and manage related administrative tasks.

Required Experience / Certifications / Licensure

• Must possess a current, unrestricted Registered Nursing license in Alaska.
• Graduate of an accredited nursing school; Bachelor's degree preferred.
• A minimum of two years of recent case management experience.
• Basic Life Support (BLS) certification preferred; National Case Management certification preferred.
• Experience in an ambulatory care setting is strongly preferred.
