LPN LTC

The Licensed Practical Nurse (LPN) provides compassionate, hands-on nursing care to adults with complex physical and medical disabilities in a long-term residential setting. The LPN supports the nursing team by delivering direct care, administering medications, and assisting with treatment plans under the supervision of an RN.

Key Responsibilities:
• Provide direct resident care and monitor health status
• Administer medications and treatments as directed
• Communicate changes in condition to the RN and care team
• Assist with care plan implementation and documentation
• Work collaboratively with CNAs and interdisciplinary staff

Qualifications:
• Active LPN license in Pennsylvania
• Experience in long-term care or disability services preferred
• Strong teamwork, communication, and clinical skills
