Job Description

We are seeking a skilled Home Health Registered Nurse with expertise in musculoskeletal, cardiac, and neurological care. The role involves conducting home health visits for a diverse patient population, implementing home exercise programs, and providing patient family education. You will assess various conditions, document progress, and utilize medical equipment in patient care.

Responsibilities

• Perform skilled home health visits for 6-8 patients per day.
• Utilize and assess adaptive devices such as gait belts, walkers, and lifts.
• Document patient conditions and progress using the Ace Timesheet documentation system.
• Educate patients and families on care plans, medications, and health management.
• Identify and respond to signs of physical, sexual, and psychological abuse.
• Ensure safety and injury prevention in home environments.

Required Experience / Certifications / Licensure

• Minimum of 1 year of home health experience.
• OASIS training preferred.
• Valid Florida state license or compact license required.
• Current BLS certification.
• Must have a valid driver's license and reliable transportation for home visits.
• COVID-19 vaccination required or valid exemption on file.
