5x8s, 8:00am - 4:30pm, no weekend or on-call requirements. Holiday coverage may be needed.

Job Description

We are seeking a skilled Outpatient Physical Therapist to provide high-quality care to our patients. The ideal candidate will possess experience in outpatient rehabilitation settings and excel in patient evaluations and treatment planning.

Responsibilities

• Assess patient needs and develop individualized treatment plans
• Implement therapeutic interventions and monitor progress
• Educate patients and families on treatment options and health management
• Maintain accurate patient records using Epic charting system
• Collaborate with interdisciplinary teams to enhance patient care

Required Experience / Certifications / Licensure

• Minimum of 1 year experience in outpatient physical therapy
• Current SC or Multi-State Compact Physical Therapist License
• BLS certification required
