This position offers a shift from 08:00 AM to 04:30 PM, with call responsibilities one day a week and every fourth weekend.

Job Description

The role requires a range of personal health history documentation and professional certifications for onboarding. This includes verification of vaccinations, health statements, drug screening, and various training and compliance forms.

Required Experience / Certifications / Licensure

• COVID-19 Vaccine

• Influenza Vaccine
• 2 Step PPD OR IGRA (Quantiferon/Tspot)
• Varicella
• Physical/Health Statement
• Trinity 10 Panel Drug Screen
• Various Trinity compliance and background verification forms
