Job Description

The Physical Therapist evaluates assigned patients, develops, implements, and modifies treatment plans in accordance with patient needs and physician direction. Responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients while ensuring functional treatment programs that facilitate effective community reintegration. The role includes serving as an educator for patients, families, students, and coworkers.

Responsibilities

• Completes comprehensive evaluations of assigned patients and develops appropriate treatment plans and goals.
• Collaborates with an interdisciplinary team to address barriers to discharge.
• Manages individual, concurrent, and group treatment sessions based on patient care needs.
• Maintains clear, concise, accurate, and timely medical record documentation.
• Participates in student programs providing a positive learning environment.
• Actively pursues professional development and advocacy efforts.

Required Experience / Certifications / Licensure

• BLS Certification Required Prior to First Shift.
• Must hold a valid, current State license.
• Minimum of 2 years of experience in a relevant setting.
