Day shift, 5x8-hour schedule (08:00 AM - 05:00 PM).

Job Description

As an Occupational Therapist in a home health setting, you will provide skilled care to adult patients with various diagnoses, including post-surgical conditions and chronic illnesses. Your role will involve assessing patient needs and implementing therapy interventions to enhance their recovery and promote independence in activities of daily living.

Responsibilities

• Conduct assessments and develop treatment plans based on patient needs.
• Implement therapy interventions including splinting, joint mobilization, and manual techniques.
• Provide education to patients and families regarding care and safety measures.
• Perform home health visits, managing a minimum of 6 visits daily.
• Document patient progress and communicate with interdisciplinary team members.

Required Experience / Certifications / Licensure

• Must have home health experience with a minimum of 1 year, 2 years preferred.
• Valid state license to practice as an Occupational Therapist in Florida.
• BLS certification required.
• OASIS training preferred.
• Experience with Cerner charting system is preferred.
