Job Description

We are seeking a compassionate and dedicated LPN to work in a Memory Care unit, providing high-quality care to residents with dementia and Alzheimer’s. The role involves a high level of patient interaction and requires adherence to standards of care within a long-term care setting.

Responsibilities

• Provide nursing care and support for residents with dementia and memory impairments.
• Administer medications and treatments as prescribed.
• Perform routine assessments and monitor patients' health.
• Engage residents in therapeutic activities as part of care plans.
• Collaborate with interdisciplinary teams to improve resident outcomes.

Required Experience / Certifications / Licensure

• A minimum of 1 year of clinical experience is required.
• Experience in dementia capable care preferred.
• Current BLS certification required.
• EPIC experience within the past 2 years is mandatory.
