Day shift, 5 days a week from 8:00 AM to 4:30 PM.

Job Description

This position is for a Home Health Physical Therapist. The manager is open to candidates without prior home health experience. The role involves conducting initial assessments, providing therapy for various conditions, and collaborating with a multidisciplinary team to achieve discharge goals.

Responsibilities

• Perform initial and ongoing assessments on patients.
• Develop and implement patient care plans.
• Provide therapy for various diagnoses including COPD, CHF, and orthopedic conditions.
• Collaborate with an interdisciplinary team to meet patient care goals.
• Educate patients and families on home exercise programs and safety measures.

Required Experience / Certifications / Licensure

• A minimum of 1 year of physical therapy experience is required.
• NH state license, or pending license with the ability to obtain by start date.
• Basic Life Support (BLS) certification is required.
• Experience with Epic documentation system is preferred.
