Night Shift: 5x8, 40 hours per week, with a weekend requirement of every other weekend.

Job Description

The Physical Therapist is responsible for providing professional rehabilitation therapy to patients in their home. This involves conducting evaluations, developing, and implementing individual goal-oriented therapy programs in compliance with organizational policies and external regulations.

Responsibilities

• Complete patient evaluations within specified time frames, ensuring holistic assessments.
• Develop individualized treatment programs based on evaluation findings.
• Maintain accurate documentation of progress notes and reassessments in a timely manner.
• Coordinate with physicians and healthcare team members for effective patient care.
• Supervise the Physician Therapy Assistant (PTA) and ensure compliance with regulations.
• Educate patients and caregivers regarding the treatment plan and home programs.
• Participate in discharge planning throughout the patient's course of care.

Required Experience / Certifications / Licensure

• Minimum of six (6) months of Physical Therapy experience.
• Registered Physical Therapist license is required.
• BLS certification must be from the American Heart Association (AHA).
