Job Description

The Home Health Nurse is responsible for providing skilled nursing care to patients in their homes. This role involves performing assessments, documenting patient conditions, and implementing care plans tailored to individual needs. The ideal candidate will have experience in home health and be equipped with a thorough understanding of various medical equipment and devices.

Responsibilities

• Conduct assessments and evaluate patient care requirements
• Document patient progress and care in accordance with established protocols
• Utilize medical equipment such as gait belts, rolling walkers, and more for patient assistance
• Educate patients and families regarding signs and symptoms, medication management, and safety measures
• Develop and implement home exercise programs and rehabilitation plans
• Ensure adherence to safety and injury prevention practices

Required Experience / Certifications / Licensure

• Minimum of 1 year of home health experience required
• BLS certification required
• Current state nursing license or compact license
• OASIS training preferred
• Valid driver's license and personal vehicle for home visits
• COVID vaccination (or exemption) and flu shot required
