Job Description

We are seeking a qualified Home Health Physical Therapist to provide skilled physical therapy services to patients in their homes. The ideal candidate will have experience with various patient diagnoses including orthopedic and neurological conditions, and be familiar with home health documentation practices.

Responsibilities

• Conduct thorough patient assessments and develop personalized treatment plans.
• Utilize a wide range of physical therapy equipment and adaptive devices.
• Provide education and support to patients and their families regarding care plans.
• Document patient progress and communicate effectively with the healthcare team.
• Ensure safety and compliance with best practices in injury prevention.

Required Experience / Certifications / Licensure

• 1 year of home health experience required; OASIS training preferred.
• Must hold a valid state license and BLS certification.
• Experience with charting systems, preferably Cerner.
