Job Description

We are seeking a Licensed Practical Nurse (LPN) for a long-term care unit. The LPN will provide essential nursing care to patients with various diagnoses, including multiple sclerosis, orthopedic conditions, dementia, and memory care.

Responsibilities

• Provide direct patient care in a long-term care facility.
• Administer medications and treatments as prescribed.
• Monitor and record patients' vital signs and health status.
• Assist in daily living activities and promote patient well-being.
• Communicate effectively with the healthcare team.
• Float within the building as required.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience in a long-term care facility.
• Current, active LPN license in New York.
• Current BLS certification.
• Drivers license with address matching demographics.
• NURSYS report for license verification.
• Two recent charge or manager references.
• Experience with PointClick Care EMR system.
