Job Description

We are seeking a skilled and compassionate LVN/LPN to join our Medical Surgical team. The successful candidate will be responsible for providing high-quality patient care, supporting RNs and MDs, and taking on patient assignments in a fast-paced acute care environment. The role involves monitoring patient conditions, administering medications, and ensuring adherence to care protocols.

Responsibilities

• Administer medications including oral, IM, SQ, and eye/ear drops.
• Monitor and document patient vital signs and changes in condition.
• Perform wound care, dressing changes, and assess pressure ulcers.
• Provide personal care including bathing, mouth care, and feeding.
• Assist in management of Foley catheters and blood glucose monitoring.
• Communicate effectively with the healthcare team regarding patient status.
• Participate in the admission and discharge processes for patients.

Required Experience / Certifications / Licensure

• Current NY State LPN license.
• 1 year of experience in an acute care setting within the last 2 years.
• Basic Life Support (BLS) certification.
• Experience with patient ratios of 1:6.
• Familiarity with Epic charting system.
