Shift: 8 AM - 4 PM

On call or weekends required: Yes – 1 weekend per month

Float required to different counties according to need.

Job Description

The Hospice Registered Nurse (RN) performs patient assessments to evaluate appropriateness for hospice care, develops personalized care plans for patients and families, and ensures the effective use of resources in accordance with compliance regulations.

Responsibilities

• Accurately assess patients’ needs and determine the appropriateness for hospice care.
• Develop and implement care plans in collaboration with the patient, family, and interdisciplinary team.
• Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement.
• Conduct discharge assessments and ensure timely documentation transfers.
• Utilize age-appropriate teaching methods for patient and family education.
• Participate in interdisciplinary team meetings and contribute to patient goal discussions.
• Follow infection control measures and promote a safe environment for patients and staff.
• Provide mentorship to new employees and support agency growth through collaboration.

Required Experience / Certifications / Licensure

• Must have 2-3 years of experience as a Registered Nurse in a home hospice environment.
• Current New Jersey RN License.
• Experience using an Electronic Medical Record (EMR) system.
• Basic Life Support (BLS) certification is required.
• Excellent critical thinking and communication skills.
