Job Description

We are seeking an LPN to provide care in a Memory Care Unit specializing in dementia, Alzheimer’s, and memory long-term care. The role requires working collaboratively with a dedicated team to support patients, ensuring a high standard of care.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor and document patient progress and respond to changes in patient conditions.
• Support daily living activities and provide emotional support to patients and families.
• Implement and adhere to unit specific procedures and protocols.

Required Experience / Certifications / Licensure

• Minimum of 1 year of clinical experience, preferably in memory care or a related specialty.
• Dementia capable care certification preferred.
• EPIC experience required within the past 2 years.
• Must pass a comprehensive background check and maintain required certifications.
