Hospice Registered Nurse
Schedule
On-call shifts required including overnights and weekends, 1-2 per month (starting at 4:00 PM and 6:30 PM).
Job Description
The Hospice Registered Nurse will perform patient assessments, develop and implement the plan of care, and ensure the appropriate utilization of services while providing compassionate care to patients and their families in a home hospice environment.
Responsibilities
- Accurately assess patient appropriateness for hospice care and document learning needs and progress toward goals.
- Develop and implement a plan of care for the patient and family, ensuring adherence to evidenced-based knowledge.
- Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement.
- Complete discharge assessments and perform data transfers within designated timelines.
- Utilize age-appropriate teaching methods for patients and families, documenting education tools used.
- Participate in interdisciplinary communication and attend bi-weekly IDT meetings to evaluate and revise care plans.
- Ensure a safe environment for patients and co-workers, following infection control measures.
- Conduct Home Health Aide (HHA) supervision as per standards and follow the scheduling process effectively.
- Participate in the Performance Improvement Program by reviewing health records and gathering data to support activities.
- Provide orientation and mentorship to new employees and cross-train to learn new skills.
Required Experience / Certifications / Licensure
- Must have a valid New Jersey RN License.
- Minimum of 3 years experience in a hospice care facility.
- Experience in a home hospice environment is required.
- Experience using an EMR and excellent critical thinking and communication skills are necessary.
- BLS certification is required.
- Valid New Jersey driver's license is needed.
