Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician directions. This role is responsible for delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that allow for effective return to community activities. The therapist also serves as an educator of patients, families, students, coworkers, and themselves, while performing professional duties related to patient care and the development of therapy services.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop individualized treatment plans focused on progress towards discharge.
• Document medical records in a clear, concise, accurate, and timely manner, complying with state and federal guidelines.
• Ensure care is provided under valid physician orders and within insurance approvals.
• Participate in the student program, providing supervision and developing learning objectives for students.
• Engage in professional development through continuing education and self-study.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
• Contribute to organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• A minimum of 1 year of outpatient clinical experience.
• Current CPR certification.

Schedule

This position involves a 5x8.0 schedule.

Assignment Duration

This is a 13-week contract position.
