Physical Therapist (Home Health) - Day Shift

Job Description

We are seeking a dedicated Physical Therapist to join our home health team. The successful candidate will provide high-quality therapy to patients in their homes, focusing on individualized patient care and rehabilitation.

Responsibilities

• Conduct patient evaluations and create treatment plans.
• Administer therapy interventions to help patients achieve their rehabilitation goals.
• Document patient progress accurately and timely using electronic health records.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Educate patients and their families about the rehabilitation process and home care techniques.

Required Experience / Certifications / Licensure

• Graduate degree from an accredited Physical Therapy Program (approved by the APTA).
• Minimum of one-year physical therapy experience in home health or facility preferred.
• Current and unrestricted Physical Therapy Licensure.
• Current CPR certification.
