Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to the patient's needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for the assigned patients and ensures the design of functional treatment programs that facilitate effective community reintegration. The therapist also serves as an educator for patients, families, students, and coworkers, and performs additional professional duties associated with patient care and therapy service development.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals focused on patient discharge within an established length of stay.
• Document medical records clearly, accurately, and timely in compliance with guidelines.
• Maintain care under signed, valid physician orders and within insurance approval.
• Participate in student programs, providing supervision and development of learning objectives.
• Create an individualized professional development plan and pursue continuing education.
• Adhere to discipline-specific regulatory guidelines and maintain professional licensure.
• Foster a culture of patient and employee safety, promoting a safety-focused work environment.
• Contribute to performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ years of outpatient clinical experience.
• Current CPR certification.

Schedule

This is a 13-week contract position with a 5x8.0 schedule.
