Job Description

We are currently seeking a dedicated Registered Nurse for our Case Management department. In this role, you will be responsible for conducting discharge planning and utilizing Interqual criteria to confirm patient status within a hospital environment. This position requires flexibility and a proactive approach to patient care.

Responsibilities

• Conduct discharge planning for patients.
• Utilize Interqual criteria to assess patient status.
• Collaborate with a team of case managers and social workers.
• Manage a patient caseload of 20-25 patients per RN.

Required Experience / Certifications / Licensure

Must have:

• Active RN license.
• Minimum of 2 years of hospital case management experience.

Preferred:

• Additional experience in hospital case management.
