Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that facilitate a successful return to community activities. Additionally, the therapist serves as an educator for patients, families, students, and coworkers and performs other professional duties related to patient care and therapy services delivery.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals focused on progressing patients toward discharge.
• Document all medical records accurately and timely, including goal writing and assessments.
• Participate in student programs as assigned, providing supervision and evaluations.
• Maintain professional licensure in good standing and complete continuing education requirements.
• Demonstrate awareness of organizational performance improvement initiatives.
• Promote patient and employee safety and adhere to safety policies and procedures.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• CPR Certification is required.
