Shift: 12 Hour Day Shift

MUST BE ABLE TO WORK ALL HOLIDAYS. Scheduling varies with no guarantee of weekends or days off unless specified in the contract.

Job Description

Seeking a dedicated LPN for our Post-Acute Skilled Nursing Rehab unit. The successful candidate will provide high-quality nursing care to patients in a post-acute setting, ensuring the best possible outcomes for our diverse patient population.

Responsibilities

• Administer medication and treatments as prescribed.
• Perform phlebotomy and EKGs.
• Facilitate patient transports and assist with care as needed.
• Manage patient care documentation through EPIC.
• Participate in patient assessments and care planning.

Required Experience / Certifications / Licensure

• Current LPN license in the state of Vermont at the time of submission.
• A minimum of 1 year of clinical experience in a skilled nursing setting.
• IV Certification preferred for LPNs.
• Experience with EPIC documentation within the last 2 years.
• Dementia capable care certification preferred.
• Prior travel nursing experience required.
