Shift: 18:30 - 07:00 (12-hour shifts); every other weekend and holiday rotations required.

Job Description

We are seeking a dedicated Registered Nurse (RN) with a minimum of 13 weeks of Epic experience, and strong inpatient rehabilitation or medical-surgical background. You will be joining a specialized team that works with patients recovering from strokes, brain and spinal-cord injuries, amputations, and complex orthopedic injuries. Your role will be crucial in helping patients achieve their highest level of recovery and transition to home.

Responsibilities

• Provide direct patient care and maintain accurate patient records.
• Assist patients with daily activities and ensure their comfort and safety.
• Collaborate with a multidisciplinary team to develop and implement individualized care plans.
• Monitor and document patient progress, responses to treatments, and any changes in condition.
• Participate in discharge planning and patient education.

Required Experience / Certifications / Licensure

• Current RN license in the state of Wisconsin.
• BLS certification required.
• Minimum of 1 year of nursing experience in skilled nursing or a 24/7 skilled care environment; preferred experience in Med Surg or inpatient rehabilitation.
