Acute Care Case Manager

This is a 13-week contract position.

Schedule

Day shift, 5x8 hours (08:00 - 16:30). Weekend requirements based on departmental needs (every other weekend).

Job Description

The Acute Care Case Manager will be responsible for providing effective care coordination, discharge planning, and utilization review for hospitalized patients. The ideal candidate must possess hospital experience with a focus on acute care case management.

Responsibilities

• Assess patients' needs and coordinate care to expedite successful discharge.
• Organize and facilitate family meetings to develop patient treatment plans.
• Evaluate patient eligibility for benefits and services.
• Ensure compliance with CMS and The Joint Commission standards.
• Independently arrange durable medical equipment (DME), SNF, and ambulance transport as needed.

Required Experience / Certifications / Licensure

• Minimum 2 years of acute care case management/discharge planning experience.
• Must have a valid California state nursing license (compact license accepted if in hand at submission).
• Previous charge experience preferred.
