Hospice Nurse

Schedule

Monday to Friday, 8 AM to 5 PM, with on-call and weekend rotations as needed.

Job Description

At HopeWest, our Hospice Nurses provide compassionate care and support to patients and families during difficult times. This role allows you to connect with the core reasons you became a nurse, embracing the opportunity to deliver meaningful, life-changing work in hospice settings. As a Hospice Nurse, you will not only manage patient care but also educate caregivers, helping them feel confident in supporting their loved ones.

Responsibilities

• Lead the interdisciplinary team in caring for patients on your caseload.
• Provide expert symptom management and critical assessments.
• Guide patients in identifying personal goals of care.
• Deliver care in diverse settings, including private homes, nursing homes, and assisted living facilities.

Required Experience / Certifications / Licensure

• Current RN License required.
• Strong clinical skills and experience in hospice or palliative care preferred.
