Job Description

The Outpatient Physical Therapist evaluates all assigned patients, developing, implementing, and modifying treatment plans in accordance with patient needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate a return to community activities. The therapist serves as an educator for patients, families, students, and coworkers while performing other professional duties associated with patient care, development of therapy services, and program delivery.

Responsibilities

• Completes a comprehensive evaluation of assigned patients and develops appropriate treatment plans and goals.
• Documents medical records in a clear, concise, accurate, and timely manner, ensuring compliance with state and federal guidelines.
• Provides care under signed, valid physician orders and within insurance approval.
• Participates in student programs by providing supervision and evaluations.
• Engages in continuous professional development and applies new information to meet organizational objectives.
• Adheres to state practice acts and maintains professional licensure in good standing.
• Utilizes organizational performance improvement methodologies to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• CPR certification.
• Active and applicable state licensure for practicing as a Physical Therapist.
