5x8.0, Monday through Friday, 32-40 hours per week.

Job Description

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans based on patient needs, goals, and physician direction. Responsibilities include delegating and directing therapy services, ensuring functional treatment program design for effective community reintegration, and educating patients, families, and colleagues.

Responsibilities

• Conduct comprehensive evaluations and develop treatment plans focused on patient discharge goals.
• Document therapy sessions clearly, accurately, and timely in accordance with clinic policies and regulatory standards.
• Participate in student programs, providing supervision and development guidance as necessary.
• Create and pursue a personalized professional development plan.
• Adhere to ethical and regulatory standards, maintaining professional licensure in good standing.
• Engage in performance improvement initiatives to enhance quality of services.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• One or more years of outpatient clinical experience is required.
• Current CPR certification.
• Active and applicable state licensure for practice.
