Job Description

The Home Health Registered Nurse (RN) will provide essential nursing services to patients within their coverage territory in Northwest Philadelphia and Montgomery County. This role requires recent and relevant home health experience, as well as proficiency in utilizing the EPIC system and completing OASIS assessments.

Responsibilities

• Conduct start of care assessments and revisits for home health patients.
• Utilize OASIS assessments to ensure proper documentation and patient care.
• Coordinate with healthcare teams to deliver personalized patient care plans.
• Monitor patient progress and make necessary adjustments to care as needed.
• Maintain accurate and up-to-date patient records in EPIC.

Required Experience / Certifications / Licensure

• Valid RN license for the state of Pennsylvania.
• Recent home health experience is essential.
• Experience with EPIC electronic health records is required.
• Proficient in OASIS documentation and procedures.
