Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that facilitate effective return to community activities. The Physical Therapist also serves as an educator to patients, families, students, coworkers, and themselves, performing other professional duties associated with patient care, development of therapy services, and program delivery.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals, focused on progressing patients toward discharge.
• Document medical records clearly, concisely, accurately, and timely.
• Ensure care is provided under signed physician orders and within insurance approvals.
• Participate in student programs, providing a positive learning environment with appropriate supervision.
• Create individualized professional development plans and actively pursue professional growth.
• Adhere to discipline-specific State Practice Acts and maintain professional licensure in good standing.
• Understand and follow organizational performance improvement methodologies to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable licensure for the State where work will be performed.

Schedule

This position involves a 5x8.0 schedule.
