Assessing Therapist - SNF/LTC

Job Description

This role will involve serving as an assessing therapist with a lighter case load while taking on some Director of Rehabilitation (DOR) responsibilities. Tasks include attending morning meetings, participating in facility meetings, and managing schedules.

Responsibilities

• Provide strong leadership and direction to the rehabilitation team (PT, OT, ST, PTA, COTA).
• Oversee and ensure compliance with PDPM processes.
• Develop and enhance rehabilitation programs for high-acuity patients.
• Foster collaboration among interdisciplinary teams to optimize patient outcomes.
• Monitor and analyze clinical and financial performance metrics.
• Ensure compliance with all federal, state, and facility regulations.

Schedule

Days: 8 hours, Monday to Friday. Times are flexible as long as they are consistent and communicated with the Director of Rehabilitation.

Required Experience / Certifications / Licensure

• 1 year of Speech Language Pathologist (SLP) experience - Required
• Experience in a leadership role within a SNF/LTC rehab setting - Required
• Knowledge of PDPM - Required
• Current Missouri State License - Required
• New graduates are NOT accepted for this position.
