Schedule

8:00 AM - 4:30 PM, 8-hour days, 40-hour guarantee. Rotating weekends and holiday coverage as needed.

Job Description

The Physical Therapist will be responsible for providing physical therapy services to clients, primarily focusing on direct patient care. Responsibilities include collaborating with team members and participating in the development of the patient's comprehensive plan of care.

Responsibilities

• Provide direct patient care in a home health setting.
• Collaborate with other healthcare team members.
• Develop and implement comprehensive plans of care for patients.
• Utilize rehabilitation equipment, including splints, orthoses, prostheses, wheelchairs, and ADL equipment.
• Apply appropriate modalities as indicated in the treatment plans.

Required Experience / Certifications / Licensure

• 2+ years of acute nursing care and home care experience required.
• Must possess a valid Driver License.
• Must have Home Health Certification.
• Proficiency with OASIS documentation is required.
• Current Massachusetts Physical Therapist License is required.
• Active American Heart Association BLS certification is required.
• Ability to work independently with minimal supervision.
