This is a 13-week contract position.

Schedule

Shift is 8:00 AM - 4:30 PM, 8-hour days with a 40-hour guarantee; rotating weekends and holiday coverage required as needed.

Job Description

The Physical Therapist is responsible for providing physical therapy services to clients of the agency. The primary function includes the provision of direct patient care, collaboration with other team members, and participation in the development of the patient’s comprehensive plan of care.

Responsibilities

• Evaluating patient needs and developing individualized treatment plans.
• Implementing and monitoring therapy interventions.
• Providing education and support to patients and their families.
• Using rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, and additional ADL equipment.

Required Experience / Certifications / Licensure

• Candidates must have 2+ years of acute nursing care and home care experience.
• Valid Massachusetts Physical Therapist License.
• Certification in Home Health required.
• Proficiency with OASIS is non-negotiable.
• Active American Heart Association BLS certification.
• A valid driver's license and personal vehicle are required.
