Job Description

We are seeking a skilled Physical Therapist to join our Rehabilitation Services team. The primary focus will be on providing high-quality physical therapy evaluations, establishing individualized plans of care, and working collaboratively with the rehabilitation team on patient progress and discharge needs. The job entails treatment in various settings, including patient rooms, hallways, outdoor areas, and our rehab gym.

Responsibilities

• Perform physical therapy evaluations and establish comprehensive plans of care.
• Assist with rounds and participate in care conferences as needed.
• Engage in frequent communication with the rehab team regarding patient progress.
• Coordinate necessary equipment and discharge plans for patients.

Required Experience / Certifications / Licensure

• Must hold a valid NC Physical Therapy license.
• Current CPR certification is required.
• Minimum of 3 years of experience in a hospital and/or nursing home setting.
