Job Description

We are seeking a dedicated Physical Therapist (PT) to provide care in a long-term acute care facility. The PT will be responsible for evaluating, developing, and implementing treatment plans for patients with various conditions, primarily adults to geriatric patients.

Responsibilities

• Conduct patient evaluations to assess their physical condition and create tailored treatment plans.
• Implement appropriate therapeutic interventions to enhance patient mobility and functionality.
• Collaborate with a multidisciplinary team to ensure comprehensive patient care.
• Maintain accurate and timely documentation of patient progress in the Electronic Health Record (EHR).

Required Experience / Certifications / Licensure

• Active Physical Therapist (PT) license in California.
• Current Basic Life Support (BLS) certification.
• Minimum of one year of experience in a Skilled Nursing Facility (SNF) or hospital setting.
• Experience with tracheostomy and ventilator patients is preferred.
