Job Description

CVHCare is seeking a qualified Occupational Therapist to join our home health team. New graduates are welcome to apply, and while home health experience is preferred, it is not required. The position is based in designated territories in Santa Clara County, including areas such as Sunnyvale, Cupertino, Milpitas, Mt. View, Campbell, Los Altos, Saratoga, Los Gatos, and San Jose.

Responsibilities

• Conduct evaluations and develop treatment plans for clients in their homes.
• Provide therapy interventions and document patient progress.
• Be available for weekend coverage and potential travel outside of the service area as needed.
• Maintain productivity requirements as specified.
• Work with Covid+ and Covid PUI patients as required.

Required Experience / Certifications / Licensure

• Current California Occupational Therapist License.
• BLS Certification.
• Valid Driver's License, Auto Insurance, and Vehicle Registration.
• TB Test (annual), MMR, Varicella, HepB, TDap (10 years), and Influenza Vaccination (annual).
• Drug Screen and Physical Examination.
