Job Description

Caledonia Home Health Care & Hospice is seeking a Physical Therapist to provide in-home therapy services aimed at maximizing the independence and safety of our home care and hospice patients. The successful candidate must be adept at working independently while also collaborating effectively as a member of our multidisciplinary team.

Responsibilities

• Provide in-home therapy services to patients.
• Develop and implement individualized treatment plans.
• Collaborate with a multidisciplinary team to optimize patient care.
• Document patient progress and outcomes accurately.
• Ensure the safety and independence of patients during therapy.

Required Experience / Certifications / Licensure

• Must have a valid VT licensure as a Physical Therapist.
• Experience in a home care setting is required.
• OASIS experience is required.
• Valid driver's license and reliable vehicle are mandatory.

Assignment Duration

This is a 13-week contract position.
