Physical Therapist - SNF

Job Description

We are seeking a skilled Physical Therapist to organize and conduct medically prescribed physical therapy services in a Skilled Nursing Facility (SNF). The ideal candidate will address impairments and functional limitations, aiming to improve the patient's quality of life following injury or disease. All assessments and treatment plans will consider the patient's gender, age, cultural differences, and educational needs.

Responsibilities

• Conduct thorough physical therapy assessments and develop individualized treatment plans.
• Implement appropriate interventions to help patients regain function and mobility.
• Monitor and document patient progress regularly, adjusting treatment plans as necessary.
• Educate patients and their families about treatment plans, exercises, and lifestyle modifications.
• Collaborate effectively with a multidisciplinary team to provide comprehensive patient care.

Required Experience / Certifications / Licensure

• Completion of an accredited professional course in Physical Therapy by the American Physical Therapy Association.
• Current licensure to practice Physical Therapy in the state of Montana and BLS certification.
• Amiable personality with sound judgment and an empathetic approach towards patient care.
• Ability to pass a background check and drug screen.

Schedule

The shift for this position is varied, with guaranteed hours of 40 per week.

Assignment Duration

This is a 13-week contract position with the possibility of extension based on performance and facility needs.
